New Patient Registration

rcgpapplestethPatients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of your name and address such as a driving licence/ passport and recent utility bill.

You will also need to complete a registration form (GMS1) and a new patient health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team.

If you would like to register at Essex House Medical Centre you will need to collect a registration pack from the reception desk.

Before we can register you, you will need to complete the GMS1 form along with the new patient health questionnaire and return this to the surgery along with photo ID and proof of address for adults or a Birth Certificate or FP68 certificate for those under 16.

Without these details we may be unable to register you with us.
